Notice of Privacy Practices

IMG Ambulance LLC — Protected Health Information

Please review carefully:
This notice describes how medical information about you may be used and disclosed and how you can obtain access to this information

Your Information. Your Rights. Our Responsibilities.

IMG Ambulance LLC is required by law to maintain the privacy and security of protected health information (“PHI”), provide you with this notice of our legal duties and privacy practices, and follow the notice currently in effect. This notice applies to PHI created or received by IMG Ambulance in connection with treatment, ambulance or transportation services, payment, and healthcare operations.

1. Your Rights

When it comes to your health information, you have the following rights. Contact our Privacy Officer using the information at the end of this notice to exercise them.

Obtain an Electronic or Paper Copy

You may ask to see or obtain an electronic or paper copy of your medical record and other PHI we maintain about you. We generally will provide a copy or summary within 30 days. We may charge a reasonable, cost-based fee as permitted by law. In limited circumstances, we may deny access and will explain any review rights available to you.

Ask Us to Correct Your Record

You may ask us to correct PHI that you believe is incorrect or incomplete. We may deny the request, but we will explain the reason in writing, generally within 60 days.

Request Confidential Communications

You may ask us to contact you in a specific way, such as at a particular telephone number or address. We will accommodate reasonable requests.

Ask Us to Limit What We Use or Disclose

You may ask us not to use or disclose certain PHI for treatment, payment, or healthcare operations. We are generally not required to agree, and we may deny a request if it could affect your care. If we agree, we may still use or disclose information needed for emergency treatment.

If you pay out of pocket in full for a service, you may ask us not to disclose information about that service to your health plan for payment or healthcare operations. We will agree unless a law requires disclosure.

Receive an Accounting of Certain Disclosures

You may request a list of certain disclosures of your PHI made during the six years before your request, including who received it and why. The accounting will not include disclosures for treatment, payment, healthcare operations, disclosures you authorized, and certain other disclosures excluded by law. One accounting in a 12-month period is free; we may charge a reasonable, cost-based fee for additional requests.

Receive a Copy of This Notice

You may request a paper copy at any time, even if you agreed to receive the notice electronically. We will provide one promptly.

Choose Someone to Act for You

If a person is legally authorized to act as your personal representative, such as under a medical power of attorney or guardianship, that person may exercise your rights. We will verify the person’s authority before acting.

File a Complaint

You may complain to IMG Ambulance if you believe your privacy rights were violated. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by calling 1-877-696-6775, writing to 200 Independence Avenue, S.W., Washington, D.C. 20201, or visiting https://www.hhs.gov/hipaa/filing-a-complaint/. We will not retaliate against you for filing a complaint.

2. Your Choices

For certain information, you may tell us how you want it shared. Tell us your preference, and we will follow your instructions when required.

Family, Friends, and Disaster Relief

You may tell us whether to share relevant information with family, close friends, or others involved in your care or payment, or with disaster-relief organizations. If you cannot tell us your preference, such as when you are unconscious, we may share information if we believe it is in your best interest and permitted by law. We may also disclose information to lessen a serious and imminent threat to health or safety.

Written Permission Required

We will not use or disclose PHI for marketing, sell PHI, or disclose most psychotherapy notes unless you give written permission, except where law permits otherwise. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

Fundraising

If we contact you about fundraising as permitted by law, you may tell us not to contact you again. If we maintain substance use disorder patient records subject to 42 C.F.R. Part 2, we will provide any advance notice and choice required before using those records for fundraising communications.

To the extent we maintain substance use disorder patient records subject to 42 C.F.R. Part 2, those records receive additional protection. We will not use or disclose Part 2 records in civil, criminal, administrative, or legislative investigations or proceedings against you unless you provide written consent or the use or disclosure is authorized by a court order accompanied by the process required by law.

6. Our Responsibilities

We are required by law to maintain the privacy and security of your PHI.

We will notify you as required if a breach may have compromised the privacy or security of your PHI.

We must follow the duties and privacy practices in the notice currently in effect and provide you with a copy.
